WS 2 - Workshop: How Collecting, Using and Sharing SDOH Data can Improve Health and Healthcare Outcomes

Many challenges and opportunities exist related to collecting, sharing, and using social determinants of health (SDOH) data. During this Workshop, speakers will identify these as well as highlight best practices for SDOH data sharing between healthcare providers and community-based organizations, specifically related to food insecurity, housing instability, transportation needs, utility difficulties, and interpersonal safety. 

Attendees will have the opportunity to discuss solutions that would allow both internal and external stakeholders to maximize the value of SDOH data and improve health equity.
